the purpose of the interface expander is to provide simple signalling to additional powerchute clients for graceful shutdown. because it is simple signaling, i am assuming it saw the UPS went to battery and it is unable to tell the difference between a self test and power failure.
before i say that though, can you confirm the cable part numbers in use on the interface expander and on the UPS serial port for the client connected there? have you reviewed the email settings to see if maybe the other client is not enabled for email to see if that is why you got an email from one client but not on the other?
the self test procedure puts the UPS on battery for about 20-30 seconds to ensure the battery is able to support the load.
