When I install the latest powerchute 5 node business edition on RH 4.8 (pbeagent-9.1.0-614.i386.rpm) it reports that the UPS communication is lost. When I set this on RHEL 5.8 server - everything works as expected (same setup, not a shared ups, no extenders, RJ45 for port on the UPS side, gave it a username and password and correct serial port).
I get this in dmesg:
ioctl32(java:20488): Unknown cmd fd(57) cmd(0000545d){00} arg(aefaed60) on /dev/ttyS0
Does anyone have the similar setup or similar problem ? I can install another version shipped in the gz archive (9.0.1) but it gives me just a simple info about the ups (that its online) and I can't configure shutdown options.

Can you verify nothing is installed in the UPS SmartSlot? This needs to be uninstalled for PowerChute and the UPS serial port to work properly on this type of UPS. Let me know what is installed first if there is something there (like a management card).
If UPS communication isnt working properly, I suggest you braindead it to properly reset it. You'll need to turn off the attached load and disconnect any communication cables from the UPS. Then, turn the UPS itself off and remove it from AC power. Once that is done, hold down the off button that looks like (o) and wait for the UPS to flash its LEDs and make an audible click. Once complete, go ahead and reconnect AC power to the UPS and turn it back on. Then you can reconnect the communication cable and your load and see if any change.

OK I've connected another SURTD3000XLI on another server with the RHEL4.8 (the same model of server, the same operating system) and it works as expected. Which leads me to conclusion either serial port is misconfigured or the cable is faulty/not appropriate (coleague of mine is certain that he used the cable shipped with the UPS).
