We recently installed a SURTA2200RMXL2U. Now the red LED (replace battery light) is on.
Should I consider this an out of the box failure?

Anytime you buy a UPS, I'd keep in mind how long it may have been sitting on a retailers shelf. Most large retailers and even APC internally swap out batteries periodically due to the fact they need to be charged every so often as part of a refresh process. This is not always the case for some retailers though unfortunately.
Regardless, I'd make sure this UPS has charged for at least 8-12 hour and has not been played with or tested (since battery capacity could be affected) and then go ahead and run another self test on it (hold down the test button on the front panel) and see if the replace battery LED clears. If it persists after the batteries charged and you just purchased it, you could ask for a warranty replacement of the batteries.
Reading your post indicates this has maybe been sitting for a month and working normally? If so, if it is under warranty, I'd probably suggest the warranty replacement of the batteries but yes, these batteries should last 3-5 years.
