COMX200 DOES NOT TURN ON

I have the following problem, I have about 16 Comx 200 installed in a project. And they are already damaging 4. These have been installed for 5 years, but it seems to me that it should last longer. Is this a factory problem?

Could you tell me more about this Com'X installation?  What is the environment like (dusty, hot, cold, etc...)?
