forCreate Droplist and Combo box in mimics or Form

Dear Experts,

I am trying to create a create a report on Geo scada without using crystal reports. I need to show few data in table where operator can select the date and time , and select the tag using a drop down list and chose the shift using a combo box.

please provide manuals or user guide to create Forms or structure in Mimics .
