I am having the "System failure detected by Surveillance " alarm message on SMART-UPS 40kva,400V. this caused the UPS to shutdown the Output Power. I have to bypass the ups by using Mechanical bypass Switch.
What could be the cause of this error message? Is there someone who had the same issue? How did you fix it?
Any idea will be much appreciated.

Hi,
"System failure Detected by Surveillance" is a general system fault. It may sometimes be caused by improper wiring during a startup, but can also appear during normal operation. It is frequently accompanied by other fault messages that will more clearly indicate the actual issue.
Resolution:
Inspect wiring if unit is a new install. Troubleshoot any other reported faults/errors. If issue persists, contact local APC support.

Thank you Bill, i tried to check the field wiring but found nothing. All three phase are okay and all are in phase sequence.
Unfortunately, we don't know what happened to the display because there is a cursor that is blinking on the screen but we can't do anything else from the display. i tried to access the UPS through the NMC, to see the logs which i am attaching so that you can see and advise me what to do.
I am accessing the NMC on the network,however when i want to check about the UPS, i am geting the screen shot attached with a message " Lost Communication with UPS"
