We have a SYMMETRA LX that fails a self diagnostics test with no other indications that anything is wrong. The MIM and RIM have green and orange flashing LED'S on both of them. System reports nothing is wrong. We put a new power module in with firmware 353, the other 3 power modules have firmware 352. Could this be the problem?

Ideally, you don't wanna troubleshoot a Symmetra unit via Discussion Boards. I recommend if you have the time, call into support, and we can grab some information and try a few things.
Please note there's a difference in manual bypass previously stated.
There are two types of manual bypass. One through the Powerview where if you toggle System Enable it will drop your load. The other is physically throwing the Maintenance Bypass breaker located to the right of the PowerView. This and toggling system enable will not drop your load.

What exacty are the fault messages you are getting ? If you go into the diagnostics menu can you see if there are any power module faults, MIM/RIM faults, battery module or communications faults ?
Otherwise, have you tried the diagnostics with one or the other of the power modules removed ? ...or tried a manual bypass-->re-boot process to cold-start the ups (this won't drop the loads) ?
