SPARK IN THE UPS( 40 kVA Smart UPS) AND SUDDEN SHUT DOWN

We have heard and seen a loud burst and spark in our 40kVA UPS , which caused sudden shut down of the UPS and all our computers  shut down suddenly without warning. What could have caused the loud spark ? What is the remedy for this ? The Fault Message states that  " Mains not available, Frequency/volt out of range"

We recently replaced the two power modules in the UPS and later installed a 45 kA TVSS and Phase failure to protect the UPS from power surges and fluctuations.  Could this have had any effect on the UPS ?

Please help...
