On Surt3000uxi ups when input available and turn on ups indicate system fault and battery bar 3rd and 5 led flashing.

I can only find:

which indicates a high output voltage (staying over 269V for several AC line cycles) - have you measured the output voltage?
I also can only find this and not the first indicator you gave me:

1
0
1
(which is Output relay fault)
