The "Uninitialized" state, quality "Device Failure", for the ODBC client driver are associated with whether or not the driver itself is running. If the driver is not running, the state will always be Uninitialised since the connection cannot be established.

Ensure that the driver is running by looking at the Modules tab of the Server Status Dialog. If the driver is not running, examine the log files for the cause of the error and rectify accordingly.
