Hi Yan,
It's not a common thing that I have seen however not knowing the configuration, if it's device specific, what was happening on the server at the time, etc, I really don't have an answer for you.
There are a few things you may want to look at:
Is it all devices?
Is it just this notification policy or action?
Is this a manually created threshold or a pre-created alarm listed under device alarm configuration?
What is the configuration of the action / profile?
Was the system in the middle of a backup or purge?
Have you tested this alarm to see if it was a one time thing?
Have you tried rebooting the server?
Is this a hardware or VM server? Type if hardware? Resources is VM?
How many nodes?
I'm guessing the server was busy but without more detail, I'm just guessing.

Hi Yan,
Another thing I should have thought of. What are the times on the devices, console, DCE, the e-mail server, etc? If they have different time zones, this could also effect the times shown.
