Issue
"Access Denied - Invalid Card Format" in live monitoring when the Door Relocks
There is no error when the card is presented and the door unlocks normally

Cause
There is some type of electromagnetic interference affecting the reader
Resolution
There are at least 2 different situations where this has been observed:
1. When using entry and exit readers that are very close together
2. A strong LED light that was close to an entry reader. In this case, there was no exit reader
The resolutions are to move/remove the device causing the interference or in the case of the LED light to ground the shield wire of the reader to the panel. This stopped the error from happening.
