Regular "Severe - UPS: Bypass not in range; either frequency or voltage"

We had a lengthy power outage last night and the UPS ran out of battery power. When power returned (about 1 hr after restart) we started getting the error "Severe - UPS: Bypass not in range; either frequency or voltage." (I have a Symmetra 8000 with web card:)

Based on the manual I think I understand the error message. However, the ups event log does not show any over/under voltage or frequency. When I check the current state page of the web interface to the ups, it shows info below. Everything looks fine.

How do I know what the cause of the error is? (Volt or Freq). Is this recorded somewhere? What is the acceptable volt & freq range?

A little more info. I checked the MIM status and see:

state=05 fan=0
100 10 00110101
Vint=244.2 00000000

So input voltage is correctly set to 240v (I think - and why would that change after a full discharge).

The Voltage status shows:

staus

Hopefully there's another clue there for someone knowledgeable.

Here are the settings you asked for:

VoutReport set to AUTO
Output freq set to 60Hz +/- 0.1Hz
Bypass set to "Go to bypass" (was in Drop Load but I just changed it)

So now, how should I interpret these? Should I set VoutReport to 240v? (I have this unit on single phase 240v). Is the output freq too narrow? Does bypass setting matter? etc

I just replaced the SYMIM 2 weeks ago, after the last one died following a power outage. Could this be a factor?

I seem to recall that PowerChute produces a graph of Voltage over time, so I installed Powerchute Business Edition. But PowerChute isn't able to detect the UPS (connected by serial port). I think my Symmetra 8000 is not on the supported device list for Powerchute...is this worth pursuing or is this just incompatable?

I widened the freq range to +/ .3 (which MAY solve the problem - but I won't chase this further)
And I turned off drop load on bypass (which caused all my messages)
