I purchased a refurbed SMT1500RM2U from Newegg and after setting it up, it has a site fault error. I tested the plug with a multimeter and it does not appear to have any issues. I tested
hot neutral (120v)
Hot ground (120v)
Neutral ground (0v).
Status via SSH says "Online, site wiring fault" which I also see in the log. The error only appears once and does not seem to repeat unless I unplug the UPS and plug it back in. There is no load on it at all right now.
So a few questions:
Does this mean it is faulty and shipped broken?
Is it still useable?
Besides returning it, is there anything I can do to try and fix it?
Pretty unhappy that a "new to me" UPS has errors.

To be more specific besides measuring the voltage on all connections. You’ll want to verify all connections are tight and the ground is present, connected at the outlet and breaker.
As a sanity check you can move the unit to other outlets in the home and repeat the test. If the problem goes away that outlet / branch circuit needs to be inspected until you identify the root cause.
If however all outlets in the home show the same problem then the possibility of a actual fault is present. You can rule this out by installing the unit at another location for a quick test.
Ask a neighbor to borrow a outdoor outlet and power the unit from there. If the problem remains then it’s more likely the unit is defective with a couple caveats.
That would be the the system is in a unknown state. If so complete a brain dead reset following this APC FAQ:

If the problem is resolved move on. If the problem is (NOT) solved by completing a brain dead reset then the last remedy is to update the UPS Firmware.
Assuming it’s not on the latest firmware release. Keeping in mind this procedure should not be initiated unless the device is under warranty!
Given, this device is used and refurbished the serial number will reflect a out of warranty period (3) years from the date of purchase.

The only procedure you’ll follow and use is via direct connection with the supplied APC Proprietary RJ50 -> DB9 Serial cable.
The UPS ID can be viewed and confirmed via LCD About. If there’s a NMC the About UPS. In Power Chute the same information is available.
Lastly, the UPS ID is imprinted on MCU chip on the main board. It will also indicate what the factory UPS Firmware was at the time of production and release.
Lastly, if a cursory validation as outlined above shows the same wiring fault. Save your time and return the hardware for another.

Well I did test the unit by plugging into a different plug in the house which is on a different circuit breaker and it had the same error. I also took my multimeter and tested both plugs for any signs of voltage irregularities, and both plugs were 100% clean. It definitely looks like the unit is faulty. I contacted Newegg and they have started the refund process as I am returning the unit. I did a factory reset on it but hopefully the vendor doesn't try to resell it again to someone else.
